Reporting Requirements

Currently funded programs are required to complete quarterly reports even if no services were performed during that quarter. These reports can be accessed below.

  • The Cities use four quarterly reporting periods. Reports are due by the 15th day of the month after the end of each quarter.

  • Quarter 1 runs from January through March
    Quarter 2 runs from April through June
    Quarter 3 runs from July through September
    Quarter 4 runs from October through December

  • A Reimbursement Request must be submitted every quarter even if no services were performed during that quarter. The Reimbursement Request and quarterly Service Unit Report are both included in the template below.
  • CITY SPECIFIC REQUIREMENTS: Many cities accept the Reimbursement Request template in their invoicing process and require no further invoicing action on your part. However, some cities have different requirements for the Reimbursement Request. Please contact the appropriate city staff for specific requirement if unsure.

  • A Service Unit report must be submitted every quarter. All cities accept this Service Unit template (below) as the quarterly report.

  • The Demographic Report and the Outcomes Report must be submitted once annually on January 15 for the previous year. These are submitted along with the fourth quarter Service Unit Report and Reimbursement Request.

Accessing and Submitting Reports

Download the appropriate reporting template and complete, including digital signature. Email the same copy to each of the cities providing funds for the program. Each individual program must submit its own reports. If corrections are made to reports after submission, be sure to notify all funding cities of the changes and send the corrected reports to all cities.
